Donna Jo Cook, 46, of Woodbury died January 11, 2012.  She was born November 13, 1965 in Woodbury, TN.  She was preceded in death by her mother, Barbara Joan Geasley.
    
She is survived by her father, Joe Cook of Readyville; brother, Zachary Cook of Readyville; sister, Kim Cook of Readyville and two nieces, Ali Jo Peckham and Mikayla Josie Cook.

The family will receive friends on Saturday, January 14, 2012 from 4-6 PM at Smith Funeral Home.  Memorial services will follow in the Smith Funeral Home chapel. Greg Mitchell will officiate.

In lieu of flowers the family requests memorial donations to the American Cancer Society.
