NOTICE TO THE PUBLIC OF FINDING OF NO SIGNIFICANT IMPACT

Proposed New Health Center for Cannon County

City of Woodbury, Tennessee

In accordance with the National Environmental Policy Act, the Council on Environmental Quality regulations for implementing NEPA ( 44 CFR Parts 1500 through 1508) and the HHS General Administration Manual Part 30 Environmental Protection (February 25, 2000), HRSA has determined that the Proposed Construction of the new Health Center for Cannon County will have no significant adverse impact on the quality of the human environment. As a result of this FONSI, an Environmental Impact Statement will not be prepared.

The Health Resources and Services Administration (HRSA) of the Department of Health and Human Services (HHS) provides discretionary grant and cooperative agreement awards to support health centers expand their capacity to provide primary and preventive health care services to medically underserved populations nationwide.

The Tennessee Department of Health has applied for HRSA grant funding. The applicant proposes to use grant funds for the construction of a new Community Health Center for Cannon County in Woodbury, Tennessee, to replace the current location. The new facility will be approximately 5000 square feet and will include clinic rooms, staff and record keeping facilities, dental facilities, and office and meeting space for the staff. The new health center will consist of a single-story building on land adjacent to the recently constructed EMS ambulance dispatch and will be located just up the street from the Hospital. The applicant has submitted an Environmental Assessment (EA) that documents impacts of the proposed action. This EA is incorporated by reference into this FONSI.

Additional project information is contained in the Environmental Assessment for this project, which is on file at the following address for public examination upon request between the hours of 8 a.m. and 4:30 p.m., Monday through Friday.

The DeKalb/Cannon County Farm Service Agency (FSA) is hosting a County Committee Election Workshop on Thursday, June 9 at 6:00 P.M. at the Cannon County Lions Club located at 634 Lehman St. in Woodbury.

The purpose of this meeting is to provide information relating to the role of the Farm Service Agency in the community and how local administrative areas are determined.

The deadline to submit nominations for the 2016 County Committee Election is August 1, 2016.
All interested farmers and producers, especially individuals or groups who represent socially disadvantaged or beginning farmers, are encouraged to attend and participate in this meeting. Nomination forms and fact sheets, in both English and Spanish versions, will be available. FSA staff will be on hand to answer questions concerning the nomination and election processes, or any other program questions that may be presented.
