SunCrest celebrates with 'Rabbit'



SunCrest's Telehealth program has experienced countless success stories in the last three years. One includes Irvin "Rabbit" Neely of Woodbury.

Neely was being treated with home care orders for hypertension and related issues.

"We worked with him on taking his weight and blood pressure daily and helped him with diet issues," said Karen Garfield of SunCrest. "He was doing much better but unfortunately had a car accident and ended up in the hospital with a broken ankle."

SunCrest's Woodbury staff continued to work closing with him during a period of rehab,

After a series of visits and a number of phone calls, Telehealth Case Manager Tanya Green, R.N. recommended that Neely made an appointment with his cardiologist who determined that he needed a renal stent.

"That seemed to address his hypertension so he will continued on service for this episode, and if all goes well, Mr. N will be taken off of Telehealth will all the knowledge and skill to care for himself. He is so pleased with his experienced that he has told his story to anyone who will listen, including the mayor," Garfield said.

Neely recently invited the Woodbury and Telehealth team to his home for some of his 'Best in 15 States' chilli.

Thanks to the Woodbury team for taking such good care of him.
