The State of Tennessee, Department of Children's Services, has filed a petition against you seeking to terminate forever your parental rights to Shayla Marie Hernandez, Miracle Hope Hernandez and Christian James Hernandez. It appears that ordinary process of law cannot be served upon you because your whereabouts are unknown. You are hereby ORDERED to serve upon Carolyn Jennings, Attorney for the Tennessee Department of Children Services, 1200 Belmont Drive, McMinnville, TN 37110, (931) 473-0191 an Answer to the Petition for Termination of Parental Rights filed by the Tennessee Department of Children Services, within thirty (30) days of the last day of publication of this notice, and pursuant to Rule 39(e)(1) of the Tenn. R. Juv. P. you must also appear in the Juvenile Court of Cannon County, Tennessee at Woodbury, Tennessee on the 25th day of October, 2016, at 1:00 P.M. for the Adjudicatory Hearing on the Petition for Termination of Parental Rights filed by the State of Tennessee, Department of Children's Services. If you fail to do so, a default judgment will be taken against you pursuant to Tenn. Code Ann. § 36-1-117(n) and Rule 55 of the Tenn. R. of Civ. P. for the relief demanded in the Petition. You may view and obtain a copy of the Petition and any other subsequently filed legal documents at the Cannon County Juvenile Court Clerk's Office, 200 W. Main St., Woodbury, Tennessee 37190.

Cannon County 911 Board of Directors

The Board of Directors of the Cannon County 911 Emergency Communications District will meet at 6:30 pm, October 27, in the conference Room of the Stones River Hospital in Woodbury, Tennessee, or such other location as may be posted at the Courthouse, where and which time the Board will consider any matter that may be properly presented. The public is invited to all meetings.
