NOTICE TO CREDITORS CHANCERY COURT OF CANNON COUNTY, TENNESSEE Estate of Bobby Jean Floyd Notice is hereby given that on the 17 day of May, 2016, lett ers of testamentary or lett ers of administration with respect to the estate of Bobby Jean Floyd deceased were issued to the undersigned by the Chancery Court of Cannon County, Tennessee. NATHAN S NICHOLS CLERK & MASTER MATTHEW COWAN ATTORNEY *************************** Health Resources and Services Administration NOTICE TO THE PUBLIC OF FINDING OF NO SIGNIFICANT IMPACT Proposed New Health Center for Cannon County City of Woodbury, Tennessee C8DCS29111, Project 129605-01 In accordance with the National Environmental Policy Act, the Council on Environmental Quality regulations for implementing NEPA ( 44 CFR Parts 1500 through 1508) and the HHS General Administration Manual Part 30 Environmental Protection (February 25, 2000), HRSA has determined that the Proposed Construction of the new Health Center for Cannon County will have no signifi cant adverse impact on the quality of the human environment. As a result of this FONSI, an Environmental Impact Statement will not be prepared. The Health Resources and Services Administration (HRSA) of the Department of Health and Human Services (HHS) provides discretionary grant and cooperative agreement awards to support health centers expand their capacity to provide primary and preventive health care services to medically underserved populations nationwide. The Tennessee Department of Health has applied for HRSA grant funding. The applicant proposes to use grant funds for the construction of a new Community Health Center for Cannon County in Woodbury, Tennessee, to replace the current location. The new facility will be approximately 5000 square feet and will include clinic rooms, staff and record keeping facilities, dental facilities, and offi ce and meeting space for the staff . The new health center will consist of a single-story building on land adjacent to the recently constructed EMS ambulance dispatch and will be located just up the street from the Hospital. The applicant has submitt ed an Environmental Assessment (EA) that documents impacts of the proposed action. This EA is incorporated by reference into this FONSI. Additional project information is contained in the Environmental Assessment for this project, which is on fi le at the following address for public examination upon request between the hours of 8 a.m. and 4:30 p.m., Monday through Friday.
