Important Information For Parents Of Children Entering 7th Grade

Has your 7th grader received all of his/her immunizations? If not, the following immunization clinic may be of help:
WHO: Students Entering 7th Grade WHAT: Cannon County Health Department Immunization Clinic WHEN: July 29th, 2010 (please call 563-4243 to schedule an appointment) WHERE: Cannon County Health Department at 301 West Main Street, Woodbury, TN 37190 WHAT TO BRING: Your child’s shot record The Tennessee Department of Health (TDOH) has issued new immunization rules along with a new Official Certificate of Immunization that is required for students entering 7th grade. In summary:
- Children entering the 7th grade must show proof of two new immunizations:
- Tetanus-diphtheria-pertussis booster (“Tdap”)
- Verification of immunity to varicella (2 doses or history of disease)
- There will now be only one official TDOH certificate of immunization that must be completed by medical providers and delivered to schools upon registration.
