Has your 7th grader received all of his/her immunizations? If not, the following immunization clinic may be of help:

WHAT TO BRING: Your child’s shot record
The Tennessee Department of Health (TDOH) has issued new immunization rules along with a new Official Certificate of Immunization that is required for students entering 7th grade. In summary:
  • Children entering the 7th grade must show proof of two new immunizations:
    • Tetanus-diphtheria-pertussis booster (“Tdap”)
    • Verification of immunity to varicella (2 doses or history of disease)
  • There will now be only one official TDOH certificate of immunization that must be completed by medical providers and delivered to schools upon registration.
